SPECIAL CONSIDERATION PROCESS FOR MISSED TESTS/EXTENSIONS
What do I do if I miss a term test?
Students who miss a test due to circumstances beyond their control (e.g. illness or an accident) can request special consideration.
1) You must declare your absence using the ROSI Absence Declaration on the day of your absence.
2) You must present your case to the Department (not the instructor) by completing a "Request for Special Consideration" form.
3) Original supporting documentation (a U of T medical certificate, accident report) is to be brought to Rose Antonio, Academic Advisor, in Rm. 3018 of the CCT Building. U of T Medical Certificates MUST include the statement "This student was unable to write the test on date(s) for medical reasons." Documentation must show that the physician was consulted within one day of the test. A statement merely confirming a report of illness made by the student is NOT acceptable.
4) You have up to 72 hours from the date of the missed test to submit your request and supporting documentation. Late requests will NOT be considered.
5) If your request is granted and a make-up test is required, you will write within 2 weeks of the original test date. Be prepared for the makeup test at any time. There is no opportunity to defer a makeup test.
The University respects your privacy. The information on this form is collected pursuant to section 2(14) of the University of Toronto Act, 1971. It is collected for the purpose of administering accommodations for academic purposes based on medical grounds. The department will maintain a record of all medical certificates received. At all times it will be protected in accordance with the Freedom of Information and Protection of Privacy Act.
What do I do if I need an extension of time to complete term work?
If you require more time to complete term work that will NOT exceed the last day of classes:
1) You must contact the instructor before the due date of the assignment and let he/she know that you are unable to submit your assignment on time.
2) You must declare your absence using the ROSI Absence Declaration on the days of your absence from school.
3) You must present your case to the Department (not the instructor) by completing a "Request for Special Consideration" form.
4) Original supporting documentation (e.g. a U of T medical certificate, accident report) is to be brought to Rose Antonio, Academic Advisor, located in Rm. 3018 in the CCT Building. If you are seeking a one-day extension, the medical certificate must confirm that you were ill on the due date of the assignment; if you are requesting a longer extension, your documentation must specify exactly the length of the period during which you were unable to carry out your academic work. A physician's statement merely confirming a report of illness made by the student is NOT acceptable.
5) The department has discretion to approve extensions of time on work until the last day of exams of the term. After this date students must petition through the Office of the Registrar.
Your responsibility
It is your responsibility to obtain the decision from your instructor. Claims that a decision was not received will NOT be considered as a reason for further consideration.
It is your responsibility to follow the appropriate procedures and submit documentation on time. Failure to do so may result in denying your request.
The administrative office verifies authenticity of medical documentation by contacting medical clinics. Students who submit forged documentation are subject to severe academic penalties.
