Important Forms and Links

Internship Course Coordinators and Administrators need to enforce the following Rules and Policies:

Needed for all Intern Students:
Work/Education Agreement Form (Safety Insurance)
Accompanying Memo
Accident Report Form (attached to memo, but only filled in in case of accident)

Liability:
All students doing work towards course credit are insured through UofT liability coverage of over $5 million dollars.

Needed for some Intern Students:
Police Check with Vulnerable Sector:
if working with children, elderly or special needs sector: Either from local police station - student needs 2 pieces of picture ID.  This can take over 2 months so plan ahead.  Students residing in Toronto must have a form from the host agency. 

Ethics Review: UofT students, depending on the nature of their project or experience, need to read and apply the rules regarding the Ethics Review of their placement work.

Safety:
- Students doing lab placements or placements involving hazardous materials must take a WHMIS course - each faculty has course offerings and varying schedules.
- Students who are even rarely in a field environment - in the outdoors, in a variety of settings - as well as the Course Faculty Coordinator, need to have read and be aware of UofT's policy on Safety in the Field.
- Safety Abroad:  Students doing internship course placements outside of Canada are required to review the information on the UofT Safety Abroad website and consult with staff regarding travel safety, immunizations, etc. individually or in special sessions. 

Immunizations: 
Some internship courses require proof of certain immunizations and TB test for all students in the course.  There are some courses where this is required depending on where a student is placed - ie school or hospital.  Policies vary by course, as well as by independent organizational requirements.

Outside Organizational Forms and Policies:
Each organization in the community can have forms and policies relating to confidentiality, medical records and immunizations, their own security check, safety and WHMIS training, etc.  Students have to agree to fulfill these requirements to be hosted at these organizations.

Research Project Proposal/Experience Summary/Job Description
In some courses, students are required to submit this early on as part of their course mark.  In some courses, prospective employers are required to submit a paragraph or letter describing what type of experience they are offering the student, and what background and skills they hope the student already has to offer them.  Organizations may want to include projected outcomes and learning opportunities for the student.
